Chronic Disease Self-Management Program
CDSMP “Living a Healthy Life” is a community-based Education Program created by Stanford University. Program participants will feel empowered to take charge of their health and lead more productive and satisfying lives. By participating in the six week workshop, participants increase their confidence and motivation needed to manage the challenges of living with a chronic health condition.
What can I expect from the Chronic Disease Self-Management Program (CDSMP)?
The education and activity course is designed in 6 week increments and focuses on individual learning to self-manage chronic health condition(s) combined with group motivation and support led by a certified instructor. Content modules include: 1) techniques to deal with problems such as frustration, fatigue, pain and isolation; 2) appropriate exercise for maintaining and improving strength, flexibility, and endurance; 3) appropriate use of medications; 4) communicating effectively with family, friends, and health professionals; 5) nutrition; 6) how to evaluate new treatments.
Are there eligibility requirements or costs to participate?
The CDSMP “Living a Healthy Life” program is ideal for older adults (60+) with a chronic disease or their caregivers. There is no cost to attend these sessions. For locations and times, call the Aging & Disability Resource Center (ADRC) at 260.469.3036.
