Options Counselor / Case Manager
Case Managers and Options Counselors report to two departments within AIHS:
CM's report to the Geriatric and Disability Case Management Department OC's report to the Aging & Disability Resource Center Both roles are similar but unique as they both establish and maintain relationships with clients and community resources, in a long-term (CM) and short-term (OC) basis. AIHS promotes dignity and independence for all older adults. Both CM's and OC's work focuses on keeping clients safe and in their own homes for as long as possible. AIHS Case Managers and Options Counselors receive extensive on-site training to achieve case management certification through the State of Indiana. At AIHS, business hours are M-F 8:00-4:30 pm with optional flex hours to allow for a 12 noon dismissal on Friday afternoons. There is no mandatory overtime, on-call or weekend work within these roles. AIHS adopted a new holiday schedule effective January 1, 2023 which includes 13 paid holidays!
Work involves information & referral, assessment, enrollment, and case management utilizing specialized software within specified timeframes to assist older adults and persons with disabilities remain safe and independent at home. Agency work hours are typically 8:00am-4:30pm Monday – Friday. Office environment and in-home client visits. Must be able to travel independently throughout assigned service area.
Required Education and Experience
Bachelor’s degree from four-year college or university; or Bachelor’s degree in any field with minimum of two years full-time, direct service experience with the elderly or disabled (experience must include structured case management duties including assessment, care plan development and monitoring); or a Registered Nurse with one year’s experience in human services; or a Master’s degree in a related field may be considered as a substitute for the required experience.
