Options Counselor / Case Manager
Case Managers and Options Counselors needed for replacement and expansion positions - salary commensurate with applicable experience - extensive training program conducted to achieve case management certification - benefits package includes health insurance, 37.5 hour work week, no weekend or on-call, and 12 paid holidays!
Work involves information & referral, assessment, enrollment, and case management utilizing specialized software within specified timeframes to assist older adults and persons with disabilities remain safe and independent at home. Agency work hours are typically 8:00am-4:30pm Monday – Friday. Office environment and in-home client visits. Must be able to travel independently throughout assigned service area.
Required Education and Experience
Bachelor’s degree from four-year college or university; or Bachelor’s degree in any field with minimum of two years full-time, direct service experience with the elderly or disabled (experience must include structured case management duties including assessment, care plan development and monitoring); or a Registered Nurse with one year’s experience in human services; or a Master’s degree in a related field may be considered as a substitute for the required experience.
