Community Health Coach - Population Health

Community Health Coach needed for positions due to growth! Salary commensurate with applicable experience. Benefits package includes health insurance, 37.5-hour work week Monday - Friday, no "On Call", and 12 paid holidays!

• Performs all activities related to population health care services to achieve program goals at main office, and/or participating hospitals/facilities, and at client locations. 
• Provide population health services (assessment, resources) to assigned clients on an ongoing and emergency basis per contracted intervention protocol, within contracted timeframe, by telephone or in client’s residence that may be accessible by public transportation.
• The program goal is to assess social determinants, to improve health status, and provide resources to prevent hospital admission/readmission.
• Must be organized and tech savvy to be able to process from multiple contracts.

Required Education and Experience: Bachelor’s degree from four-year college or university. Knowledge of medical terminology and/or experience in a customer service position in healthcare setting.  Internship or practicum in related field as part of bachelor’s degree requirement from four-year college or university.
