Case Manager

Must possess degree from 4 year college or university in one of the following areas only: social work, gerontology, sociology, counseling, psychology, human services, or possess a valid RN license (4 year) as required for case management certification, or possess Master's degree in a related field meeting requirements outlined by the Division of Aging. NOTE - case management certification process will only accept the areas listed - no exceptions.

Prefer 2 years experience in case management including assessments; will consider practicum/internship experience.

Work involves structured case management utilizing specialized software within specified timeframes to assist older adults and persons with disabilities remain safe and independent at home. Agency work hours are typically 8:00am-4:30pm Monday - Friday. Office environment and in-home client visits. Must be able to travel independently throughout assigned service area.

Job Type: Full-time; 37.5 hours per week

Salary: $32,500 / year

Required education: Bachelor's in one of the areas listed above
