Case Assistant

Serves as a support for the Case Managers and Case Management Supervisors.  The responsibility will be to support the teams with client-related needs by providing communication and follow up in a timely manner to enhance quality care management services.  The Case Assistant will work directly with the client, family, Case Managers, and providers.

Required Education & Experience

Associates Degree in social work, or related field and two years of experience and/or additional education in social work or related field. High school diploma, or general education degree (GED) and four years of related experience and/or training in healthcare and/or customer service position utilizing advanced technology tools including customized software systems.
