HOSPITAL 2 HOME
Hospital 2 Home (H2H) is a grant program funded by the Administration for Community Living for individuals with Alzheimer's Disease and related dementias and their caregivers. The 30-day program provides assistance to bridge gaps in care, reduce caregiver burden and provide resources to keep the person with dementia at home and out of hospital settings. ​
