The Community Health Worker provides population health services to assigned clients on an ongoing and emergency basis per contracted intervention protocol, within contracted timeframe, by telephone or in clients' residences.

This is a full-time position working remotely.

Required Education and Experience
Associates Degree in social work, or related field and two years of experience and/or additional education in social work or related field. High school diploma, or general education degree (GED); and four years of related experience and/or training in healthcare and/or customer service position utilizing advanced technology tools including customized software systems.
