ADVANCE CARE PLANNING

Advance Care Planning is the process by which you determine your wishes in regard to medical care and treatment including the designation of a health care representative. These wishes can be expressed through several types of legal documents.

What is Honoring Choices® Indiana?

​

Honoring Choices® Indiana is an initiative committed to promoting and sustaining advance care planning (ACP) across the state to ensure individuals’ future health care preferences are discussed, documented, and honored. There are many organizations throughout Indiana that are in various stages of advance care planning efforts, and have joined together to accelerate awareness, systems changes, and policy related to ACP. Collectively, the partners that comprise Honoring Choices® Indiana – including hospitals, health systems, community groups, educators, and faith-based organizations—are helping fulfill the vision that everyone in Indiana receives care that honors personal values and goals in catastrophic or end of life care.

Honoring Choices® Indiana is part of a 13 state network that shares information, resources, and best practices. Aging & In-Home Services of Northeast Indiana in Fort Wayne is serving as the initial coordinator and fiscal agent for Honoring Choices® Indiana.
